TOTTINGTON MEDICAL PRACTICE; PUBLICATION SCHEME This Publication Scheme provides a guide to the services and practitioners who practice together within the Tottington Medical Practice as required by the Freedom of Information Act 2000 It is based on the model publication scheme for general practices produced by the NHS Freedom of Information (FIO) project board and the British Medical Association. PART ONE: Introduction This Publication Scheme is a complete guide to the information routinely made available to the public by Drs Martin, Cornmell, Kirkham, Stokes, Watts, Livingstone and El-Jouzi. It is a description of the information about the Tottington Medical Practice and the General Practitioners that work there that is available to the public. It will be reviewed at regular intervals and we will monitor its effectiveness. Your rights to information The Freedom of Information act 2000 recognises that members of the public have the right to know how public services are organised and run, how much they cost and how the decisions are made. From January 1st 2005 the FOI Act will oblige the General Practice to respond to requests about information that it holds, and is recorded in any format and it will create a right of access to that information. These rights are subject to some exemptions, which have to be taken into consideration before deciding what information it can release. In addition to accessing the information identified in this Publication Scheme, you are entitled to request information about Tottington Medical Practice under the NHS Openness Code 1995. Under the Data Protection Act 1998, you are also entitled to access your clinical records or any other personal information held about you and you can contact the Practice Manager at Tottington to do this. New environmental information regulations may be introduced as early as 2003. These will enable similar access to environmental information as under the Freedom of Information Act 2000. How much does it cost? Publications are all free unless otherwise indicated. Where there is a charge for providing information the cost will be calculated as set out in each class. Our commitment to publish information excludes any information, which can be legitimately withheld under the exemptions set out in the NHS Openness Code or Freedom of Information Act 2000 Where individual Classes are subject to exemptions, the main reasons are the protection of commercial interests and the protection of confidential personal information under the Data Protection Act 1998. This applies to all Classes within the Publication Scheme. The information included in this Scheme is grouped into 7 broad categories. It supplies services in accordance with a Personal Medical Services contract held with Bury PCT. The GPs employ two part time nurse practitioners, two part time practice nurses and a Phlebotomist There is also a health care assistant attached to the practice The practice is also a training practice for potential GPs in training and newly qualified doctors embarking on postgraduate training. You will be informed on arrival at the surgery if a student is with the Doctor. If you do not want the student present you need to inform the receptionist and GP. This practice aims to follow national and local best practice guideline, including the NICE best practice guidelines and the National Service Frameworks. Some information will be withheld including personal, confidential information about individuals that is protected by the Data Protection Act 1998. Our Services A full range of personal medical services Child health surveillance, minor surgery, contraceptive services, obstetric services and well persons clinics. Dietary advice and weight checks Travel health advice and vaccinations Womens health smears, HRT checks etc., Asthma, diabetic and heart disease clinics Smoking cessation Family planning clinic Anticoagulant clinic Young persons health advisory clinic Support for patients with drug related problems The practice is open weekdays from 8.00 a.m. 6.00 p.m. If you need medical advice outside of normal surgery hours you should contact NHS Direct on 0845 46 47 or Bardoc(Bury and Rochdale Doctors on Call) on 0161 763 4242 Members of staff conduct consultations in English. If translation services are required please contact the surgery in advance. Dr. Martin is involved in Cardiobetes, Family Planning and PCT work Dr. Cornmell is involved in Family Planning training, GP registrar training and Nurse cytology training. Dr. Watts is involved in Registrar training and pre reg House Officer training Dr. Kirkham is involved in Child Health, Immunisations and Drug and Alcohol services. Dr. Stokes is involved in Child Health, Immunisations, he is a scheme organiser and a director of B.A.R.D.O.C Dr Livingstone is involved with Family Planning. Dr. El-Jouzi has a keen interest in minor surgery and will become a trainer for our GP Registrars. Financial and funding information Tottington Medical Practice receives money from Bury PCT according to its contract with Tottington MP in exchange for services provided for patients. The income that was received from the NHS for 2001/2 is £465,119. All of these publications are free of charge. Complaints The practice follows the NHS complaints procedure and follows a strict protocol when dealing with all complaints. These charges will vary according to how information is made available.
